Must be submitted no later than March 1, 2019
In order to send your completed proposal to us, please do the following:
1. Click Download File below. (If it opens in a new window save it to your computer.)
~or~ Right-click on the document, then "save as" and save the document to your computer.
2. Open the file, complete the form, then save again adding your name to the file name.
3. Email the document (by attaching your file) to [email protected]
~OR~
4. Send the proposal via snail mail. (Address shown on the form.)
You will need Adobe Reader 9 or higher. (Available free at http://get.adobe.com/reader/.
Please note: uncheck the "McAfee Virus Scanner" box or it will download also.)
