Fiber Daze Financial Assistance
The Fiber Folks of SW MO has set aside money to provide financial assistance to encourage more people to attend some of the fabulous classes at Fiber Daze. There's an adult category and a youth category. The simple and easy application is attached. You should also complete a registration form. Follow email or US mail options on registration form. Applicants do not need to be a guild member. If you've already registered, that's okay too. The deadline is August 27th.

Registration Form Download Instructions:
1. Download the form:
a) Right Click the Download File link below.
b) Save or save link as to your computer.
2. Fill out the form in one of the following ways:
a) Open the file, then fill the form out on your computer.
~OR~
b) Open the file, then print the form and fill it in by hand. (Please do not use Pencil or Red Ink)
3. Sign the form in one of the following ways:
a) If you filled out the form on your computer:
i) Digitally sign the form (click the signature box and it will open a window, enter your
signature in this new window, choose a storage area on your computer, add your name
to the file name (YourName_fiberdazeclassregistration_2016.pdf), and click OK or
Finished. Click the signature box and your certified signature should now appear.) You
now need to save the file, because you made changes to it when you added the signature.
Make sure the file name is YourName_fiberdazeclassregistration_2016.pdf.

ii) Print the form you filled out on your computer and sign it by hand. (Please no Pencil or Red Ink)

b) If you filled out the form by hand, sign it by hand.(Please do not use Pencil or Red Ink)
4. Send the form to us in one of the following ways:
a) Email the registration form (if you hand-signed the form, scan it back to your
computer to email it):
i) If you haven't already done so, save the form with your name added to the
beginning
of the file name (YourName_fiberdazeclassregistration_2016.pdf).
ii) Put Your Name-FDREG in the subject line of the email.
iii) Attach the registration form (by clicking the paper clip at the bottom
of your email box and selecting the form or dragging the form to this
paperclip).
iv) Send it to [email protected].

b) Send by US Mail your printed and signed registration to the address shown on the form.
